ANiC parishioners’ survey

ANiC’s development team has created a survey that we’d love to have completed by all ANiC parishioners 18 and older.
The survey takes will take about 5 minutes of your time.
If you wish to submit the survey anonymously, simply complete the survey in two steps.
1. First, complete sections 1-6 (but not section7) and submit the survey.
2. Then take the survey again, completing and submitting only section 7 (but leaving sections 1-6 blank).
Confidentiality of all information will be carefully guarded.
This survey will be open from April 17 until June 3, 2011.[Please note the deadline has been extended]
